Faculty member prepares:

Supervisor drafts evaluation letter and sends it to the Department Chair/Division Director for review. | Supervisor and faculty member meet to discuss evaluation letter and finalize goals and annual assignment. | Complete evaluation packet due in Library Human Resources Office.
